Accessing Your Health Records

Under Texas law (Section 181.105 of the Health and Safety Code), you may request access to your health care records
maintained by this practice. Requests are processed in accordance with applicable state and federal law, including
timelines and permitted fees.

Psychotherapy notes are kept separately from the medical record and are used solely to support the therapist’s clinical thinking and treatment planning. These notes reflect the therapist’s internal process and are not co‑authored, not automatically accessible, and not written for an outside audience. Because psychotherapy notes contain sensitive impressions and reflections intended only for clinical use, they are not routinely released.

In accordance with ethical standards and HB 4224, information is released only when medically necessary. When a request is made, psychotherapy notes may be redacted to protect the confidentiality of the client and others, or a treatment summary may be provided when appropriate. Fees:

Requests for any portion of a client’s health care record—including psychotherapy notes, redactions, or the preparation of treatment summaries—require administrative and clinical time. Standard administrative fees will be applied and will be discussed before any materials are released.

To request your records:

  1. Submit a written request through the form on our Contact page. Please include your full name, date of birth, and a
    description of the records you are requesting.
  2. Specify your preferred method of delivery (PDF via email, mailed copies, etc.).
  3. We will respond and provide access as required by law.
